  • 1. The Customer hereby certifies the information set out in this credit application to be correct and grants Madden Materials & Transportation authorization to verify the information given.
    2. Terms are stated on all invoices presented for payment.
    3. A late charge of one and one half percent will be charged on all past due invoices not paid in accordance with terms.
    4. A service charge of $35.00 (thirty-five dollars) will be charged for any unpaid checks presented to bank for payment.
    5. Sales Tax at the rate of 8.125% will be charged on all taxable sales unless a Tax Exempt form or Texas Sales and Use Tax Resale Certificate is supplied to Madden Materials & Transportation.
